There's a place after you go through the teleporter that has 4 broken heads. The chest is right in the middle of the heads.

man, I looked all over for buttons when I was there... never saw it. is it in the actual vault area, or outside of that?

There is a dead end 2x1 on the far left, on first tile there is a switch, on the 2nd it opens a new room with the cuirass.

Meteor shield is in the Sewers, in an alcove in a 3x3 room. Picking it up closes the room off and the Leprechaun appears with a heap of firebombs. It also releases the Fire elementals into the Sewers (one near both exits to the room).

Rogue chest, Twigroot Tunnels, secret door inside the easternmost warg pen in the Nest.
