I am a PnP GM myself ;-).

You are right. However, part of the fun of such an expanded disease system would be trying to guess from the symptoms what kind of disease the character has, and give proper treatments. But this is only one system among many I would like to add, although this one wasn't in the lost continent 1, so I would have to think it and code it from scratch, which will be fun.
And INT will be important to all players, because with the learn from failure system (see TLC), intelligence is a big XP gain bonus. So an idiot won't gain XP as fast as a clever character.

I'm new here so not really in the know concerning the modding, but what I played of your mod till now is really interesting, and I dabble somewhat in the art of writing so I'd be happy to help a little

I would love to read you. PM me any background ideas, quests plots, tribes ideas, twisted grand ideas, maps of undiscovered continent... you can think of :-)
