I don't want to rule out the possibility that I have compared my list with gamebanshee, and -seeing that gamebanshee had items listed I had not- added the arrows to my list.

It was a long night, and it took endless hours to run through all the notes (I had written down every item, including herbs and stuff), so it might very well be possible.

I'll double-check it on my next run, though.

Not true. The skeleton archers can drop the arrows. They won't drop it every time. You can save, kill a skeleton archer and watch it drop the arrow. Then you can load it, kill it again, and this time it might not drop an arrow.

Does one have to keep all the notes in the inventory in order to complete the title achievement? Or does the discovery only count towards the title achievement and one can basically throw them right away after reading them?

Picking up a note will update your counter, no matter what you do afterwards with it

I didn't bothered to read all the posts, so if this was alredy told, nevermind.

I just noticed on the maps that the level where you fight the undying has three locked doors. Well, I just found two keys and opened the three doors using different saves, just to check what's inside. One of the doors has a sack with healing potions and maybe torches, I dont remember very well.
Another has a rod of lightning,the same you can find early in the game, and the third has four lightning bombs. I stick with rod and bombs, but wasn't necessary anyway.

Yeah, you are right about that.
There are 3 rooms with 1 lightning rod, 4 healing potions and 4 lightning bombs respectively.
But they aren't exactly secrets, so I didn't mention them. Good point, though, because I do think they are quite important, at least the extra lighning rod.
