Looks like a case mismatch. Make sure the file is called "bridge_stepb.model" and not "Bridge_Stepb.model" or "bridge_stepb.MODEL" or anything like that.

Found a major bug in the new version: the hidden pressure plate at 19, 12 on level 4 gives you another 500 xp every time you step on it, so you can get infinite experience.

This happens because you're destroying the item as soon as the altar is triggered. When a thrown item lands, it plays its impact sound, but when it lands on an altar, it will trigger that altar's connectors first.
