Yeah, I'm pretty sure that all of the achievements of that sort - landing 250 unarmed hits, opening 10 iron doors, finding 5 skulls, etc. - have to be done within a single playthrough.

Try running DXSETUP.exe; you'll find it in C:\Program Files (x86)\Steam\steamapps\common\Legend of Grimrock\DirectX.

This might be because you set the Rendering Mode to Low, which is causing crashes in some Windows machines.
